Responsible in providing transition services to students under the five categories of Job Exploration Counseling, Workplace Readiness, Counseling on opportunities for enrollment in postsecondary education, Instruction in Self-Advocacy, and Work-Based Learning Experiences. Provide services to prepare consumers for Vocational Rehabilitation services, Universities, or trade schools, and/or the workforce. Assist consumers in applying for BDDS or Medicaid Waiver, Vocational Rehabilitation, or other programs to benefit students in their transition after school. Services are provided to students ages 14-22. Responsible for profitable operations and quality services.
Specific Areas of Responsibility
To provide transition services to include Job Exploration Counseling, Workplace Readiness, Counseling on opportunities for enrollment in postsecondary education, Instruction in Self-Advocacy, and Work-Based Learning Experiences
Provide program management services such as portfolio requirements prior to exiting as well as all other required forms.
Maintain accurate and thorough forms, records, and reports as required or, at minimum, weekly.
Provide transportation and personal assistance to consumers as needed and approved, to achieve goals under the five categories in which we service.
Function with a flexible schedule that is based off assigned school hours and/or students specific schedules.
Demonstrate knowledge or ability to learn about services, resources and regulations related to achieving transition outcomes to include Social Security, Medicaid, and other related plans as appropriate.
Ability to work with consumers experiencing a diverse range of abilities.
Provide coverage for other staff as schedule dictates.
Environmental Conditions
Exposure to noise, heat, cold, humidity and other environmental conditions is expected. Time is spent driving to assigned schools, local businesses, colleges, etc. Must be comfortable in a these settings.
Equipment operated
Standard office equipment including personal computers and cell phones. Additional technology as needed to meet student needs. Operates personal vehicle or arranges own transportation.

Position Requirements
A bachelor’s or associate degree is not required for this position but is a plus. High School Diploma/GED is required. Work experience with individuals experiencing a disability as well as experience in a variety of employment settings is preferred. Requires effective communication skills, both verbal and written. Driver’s license, good driving record, auto insurance meeting Corvilla’s coverage requirements and reliable transportation is required. No evidence of illegal drug use or drug abuse. No evidence of criminal activity involving a dependent population or any violent criminal activity. Bending, walking, and stooping, as well as lifting up to 50 pounds, to assist students as needed. CPR/1st Aid & Mandt certifications required.
