LPN
PURPOSE
The LPN works under the RN consultant to coordinate all medical aspects of the clients served; monitoring and assuring compliance with ICF/MR and licensure standards in all settings. Participates in interdisciplinary process and provides staff training.
SPECIFIC AREAS OF RESPONSIBILITIES
• 24 Hour crisis intervention on call and additional on call responsibilities per Corvilla policy.
• Participate in Quarterlies and Annual Resident Reviews
• See each client every week
• Physical Assessment quarterly on each client
• Instruct Medication Administration Inservice
• Medical Records review monthly with Pharmacy
• Medical Quarterlies – Document and get Dr. and Pharmacist to sign
• Pharmacy review with Pharmacy Consultant
• Administer Staff TB Vaccinations
• Administer Resident TB Vaccinations
• Participate in House meetings
• Attend Dr. appointments with clients as needed
• Monthly check supply of meds from Pharmacy into home
• Get Dr. orders as needed for IDT recommendation
• Follow up on all accident/incident reports
• Keep Monthly nursing notes, update weekly
• Medical Plan of Corrections for State
• Supervise Health & Wellness Coordinators
REPORTS TO Director of Residential Services
POSITION REQUIREMENTS
LPN License and 2 years practical experience. Excellent verbal and written communication skills and previous supervisory experience.
